Both of the following documents will need to be signed by hand. The affidavit will also need to be notarized. Your bank or a notary service in your area--such as a UPS store--can usually provide this service. 

You will need an IVP card in order to work for our network of schools. If you do not already have one, you can schedule an appointment through Fieldprint (https://fieldprintarizona.com). If you are coming from out of state, schedule your appointment for as soon as you will be arriving in Arizona. You can reschedule your appointment if needed. For teachers, please choose IVP paid employee and charter school instructor. For all other positions choose non-certificated personnel.
